What are the main differences between a Vertical vs. Horizontal Incision for Fibroid Surgery?

When considering fibroid surgery, there are three main differences between a vertical incision and a horizontal incision.

  • First, cosmesis plays a role, as a horizontal incision, also known as a bikini incision, can be more easily hidden under clothing compared to a vertical incision that is more visible, especially when wearing swimwear.

  • Second, a vertical incision tends to be more painful post-operatively than a low horizontal incision.

  • Third, a vertical incision can take longer to heal than a horizontal incision.

The choice between these incision types depends on several factors, but a vertical incision is often used when there is a need to visualize the pelvis more thoroughly. For instance, if the patient has extremely large fibroids or numerous fibroids protruding from the uterine cavity throughout the pelvis, a vertical incision may be necessary to carefully locate and remove them completely.

The preference for incision type can also vary geographically. In the United States, horizontal incisions are more commonly used, including for C-sections, whereas in other parts of the world, vertical incisions may be more prevalent. It's important to discuss your options with your surgeon to determine the best approach for your specific situation.
